Wysokodawkowa szczepionka przeciw grypie jako filar prewencji sercowo-naczyniowej u seniorów – analiza badania FLUNITY-HD oraz aktualne rekomendacje

High-Dose Influenza Vaccine as a Pillar of Cardiovascular Prevention in Older Adults: Analysis of the FLUNITY-HD Study and Current Recommendations

Summary: Influenza represents a significant global health challenge, causing approximately one billion illnesses and hundreds of thousands of deaths from respiratory complications annually. The elderly population is particularly vulnerable to severe infection due to the phenomenon of immunosenescence and co-existing multimorbidity. Influenza complications in older adults extend beyond the respiratory system, leading to organ decompensation, exacerbations of chronic diseases, and permanent loss of functional independence. The influenza virus acts as a major trigger for acute cardiac events, increasing the risk of myocardial infarction nearly tenfold in the first week of infection. The pathophysiological mechanisms of these complications involve a systemic inflammatory response, atherosclerotic plaque destabilization, and the induction of a prothrombotic state. A dedicated solution for seniors is the high-dose (HD) vaccine, containing four times more antigen than standard preparations, which allows for overcoming the weakened immune response. The breakthrough FLUNITY-HD study, conducted on a group of over 460,000 people, confirmed the high effectiveness of the HD vaccine in reducing cardiovascular hospitalizations. It was demonstrated that using the high-dose preparation reduces the frequency of hospitalizations due to heart failure by over 21% and due to laboratory-confirmed influenza by nearly 32%. Both Polish and international scientific societies recommend the HD vaccine as the first-choice preparation for individuals aged 60 and older. In Poland, seniors over 60 can benefit from a 50% reimbursement for this modern preparation, which can be administered concurrently with other vaccinations.
